Somatoform Disorder ( Help! Whats Happening To Him! )

specific symptom niya

dizzy 24/7
lighheaded
heavy head
tightband in the head and neck
dazed eyes
tingling sensation
twitching muscles
weakness
headache
internal tremors
shaking legs when walking
feeling faint or out of balance he fainted once and bumagsak na siya ng three times
hot flushes and called flushes through out his body
looking pale
fatigue

  • ^^ It is hard to diagnose when we don't see the patient. It would be best for you to go to a trusted neurologist since most are neurologic in nature.

gagaling lang namin ng hospital pero internal medicine lang muna. mag m.r.i siya ng head without contrast and m.r.i ng leeg with out contrast.pero she thinks walang problema sa ulo niya. Her initial diagnosis daw niya ay vertigo or any ear problems.
  • Some symptoms are present in MS, but some are non-specific as well. Vertigo and ear problems can present with headache, dizziness, etc and even cardiovascular problems can present as such. A cranial MRI will definitely help in telling us if this is a demyelinating disease (possible MS) or not. The muscle twitches and tremors are usually due to peripheral nervous system disease, but it can be non-specific as well. I suggest if you have a normal MRI and all the other tests like ECG, blood exams, etc are normal as well, then it would be best to go to a neurologist for further assessment.
  • What if doc walang makita sa m.r.i sa ulo niya?spinal tap ba siya?at kung wala ano kaya probable cause ng mga ganon niya?auto immune disorder na ba?hormone imbalance?thanks doc

At this point, every possible diagnosis will be discovered by exclusion. There are a whole gamut of diagnostics that can be use. But it has to be ordered in a stepwise rational order as some, if not most, of them are invasive and expensive. In the absence of any evidence that he has any organic problem, then a non organic cause (psychiatric/behavioral) cause may be considered.

I know that things are already frustrating on your end. Do not expect your doctor to find the solution at once. Shifting from doctor to doctor might not help. Eventually the last doctor you will find will end up to be the best for you only because all other tests and exclusion has been done by your previous physicians.

thanks.dati kasi pumunta na siya ng psychiatrist di man lang siya tinignan or ni rule out muna kung may underlying disease siya bigla nalang siya niresetahan ng gamot.ganon ba talaga mga pscychiatrist dito sa pinas?kasi sa US niruruleout muna nila bago sila pumunta sa conclusion ng anxiety disorder or panic disorder. at talaga ba puwede yung anxiety or panic attack symptoms last all day everyday?.pagkagising na pagkagising niya yun na kaagad nararamdaman niya.wala naman daw siya iniisip di naman daw siya anxious at nakita ko naman di naman siya naghahyperventilate e.kasi yun pagkaalam ko sa anxiety and panic attack episodic lang.sa kanya more on sa ulo talaga.eto bang nararamdaman niya puwedeng MS.gaano ba yung progress ng MS at flare up ba talagang tumatagal?thanks again
  • ^^ Psychiatrists here also do that. On their behalf, maybe, they are giving the medicines for symptomatic relief. MS can present in various ways and can progress in 4 different ways. The laboratory results will definitely help in ruling out other possible diseases.

@kram thanks doc.Ano yung 4 different ways na tinutukoy mo?ano ba mga laboratory exams para sa MS? puwede kaba magka MS kahit ok yung blood exams mo?naghihintay pa kasi siya ng appointtment para sa m.r.i niya e.what if kung ok m.r.i niya ano tingin niyo sakit niya?auto immune?or psychiatrict?thanks doc again.
  • ^^ There are 4 types of progression in MS and only the pattern of progression is described in each pattern. You can have MS even with normal examinations but in MS, there has to have 2 or more attacks. It is hard to say what he has because it is necessary to examine the patient personally. I can only assume what he has since I haven't seen him.

doc walang nakita sa m.r.i niya mri with and without contrast na ginawa sa kanya so ano kaya ang sakit niya?lupus naba,MS, or anxiety?
  • Good news! It is not MS if it showed a normal MRI. Lupus can be checked via a blood examination and anxiety is always a possibility. It is important for your bf to talk with your physician regarding the result and the other possible causes of his symptoms. It is essential to have an open mind and to trust your physician. I know it is frustrating but I'm sure your physician is frustrated too. Since your bf has a wide variety of symptoms, it is necessary to rule out each one, especially the more serious ones. Whatever the diagnosis is, hopefully he will recover from it.
  • Too much worry will create those many symptoms.

Everytime we worry. We are programming our subconscius mind that we are really sick. But physically its not. So, the more you worry. The more you feeling sick.

    Try nyo Dr. Randy Dellosa.

Thanks doc. Good news nga para sa akin yun. Tinanong ko siya kung masaya siya sa naging resulta na walang nakita sa kanya and he said 50/50. Masaya na walang nakita on the other hand daw hanggang ngayon di pa rin niya alam ng cause kung bakit siya nagkakaganon. Nakakafrustrate na raw at depressive na raw nararamdaman niya. Sabi niya kanina pag medyo daw kulang tulog niya grabe daw sumasama pakiramdam niya dati naman daw hindi siya ganon. kahhit 2 days siya walang tulog kaya niya ngayon daw kahit nakatulog na siya ng 5 hours iba daw pakiramdam niya yung feeling daw niya out of control na siya sa katawan niya na nagiging restless siya kahit humiga at matulog ulit siya.weird daw yung feeling niya sa dumadaloy sa katawan niya at lightheaded siya.kelangan daw sa kanya 10 to 12 hours of sleep palagi para medyo guminhawa pakiramdam niya. naawa na ako sa kanya kasi physically nakikita ko na namumutla talaga siya at talagang pumapayat na siya ang doctors said wala daw problema. walang problema blood pressure niya di siya anemic.so what is the root cause?
  • Thanks pero sabi niya di naman daw siya nagwoworry bigla bigla na lang daw talaga umaatake pagkagising na pagkagising niya even in his sleep umaatake raw.kasi minsan habang natutlog kami at binantayan ko yung pagtulog niya all of a sudden bigla na lang nagjerk katawan niya.pagtapos non nagising na siya and then umatake na at sobra daw ang hirap lalo na yung hilo talaga.
  • ^^ Only your physician can say what he has, he/she is in a better position than I am. We can only assume the diagnosis. If all else fails, do try to seek a psychiatrist. I'm not saying that he is psychotic, but the psyche is still part of medicine. Try to be open minded in the fact that a psychiatrist will help in the management of his case.
  • so he is suffering for anxiety disorder? pero sabi niya hindi naman daw siya nagaanxiety na tipong nahihirapan huminga at kung anxiety daw bakit siya nahimatay na?ang hirap daw na hindi alam yung sakit niya.ang worst pag kahit nakatulog siya ng 5 hours nanghihina parin siya kelangan daw mga 10 hours or more ang tulog niya na hindi naman daw pwede.saka galing na daw siya noon sa psychiatrist and said he daw cause ng anxiety ang mahimatay so di niya alam.ok naman mga stress test niya and mri.so hindi na niya talaga alam.his been frustrated na.at malamang tatamaan na siya ng depression or tinamaan na siya i don't know
  • ^^ It can be another psychiatric disorder with the same presentation. The loss of consciousness can also be due to other conditions such as heart or brain problems. It would be best if he follows up with his physician and tell the physician that he is not feeling any better. A good and trustful relationship with the physician is one way to help diagnosing the case.
  • ilynreal: Then go to a more trusted physician. It should not stop you from knowing the diagnosis. One can have a brain problem with a normal MRI and stress test is for the heart, not the brain. There are many physicians capable of handling your boyfriend. Most of them trained locally and abroad for these kind of cases. It's just a matter of going to a more trusted and respected institution/physician.
