Two different meanings in two different contexts. When a person that you think could be a potential lover says this it means that they are not physically attracted to you at all. If it happens in the first few minutes then it means that they have decided this at the intuitive level.
In a general context not involving romance, a person that says this to you means it at an intuitive level, which is rare. Sometimes, it takes just a few seconds to intuitively know a person, other times it take decades to know someone from experience.
Of course, it could be a fake friend but then again they wouldn't use the word "best". Best is a strong quantifying adjective.
