Changing TIN status from Self Employed to Employee of [X] company

Hello everyone,

Does anyone know the procedure of changing my status from 'Self-Employed Individual' to 'Employee of [x] Company'? Would there be any requirements to change my TIN status, as well as other govt accounts? (SSS, Philhealth, etc.) would there be a form or process to do this? Is it also possible that my employer do this for me?

I used to do freelance projects and now am going to a full-fledged office and would like some help.

1. saan ka ba nagbabayad ng tax dati?
2. same din ba yung rdo (revenue district office) ng magiging employer mo?

IF SAME lang din...

1. fill up this form with complete information and signature
2. tapos punta ka dun sa bir, para ipa-update ang record mo.

- pero sa ibang company, yung employee nagfifill up na then yung employer na ang bahalang magpa update

IF HINDI

- gaya ng nangyari sakin, ang ginawa pinapunta muna ako sa una kong RDO para ilipat or tanggalin ang name ko sa database nila.

-then pumunta ako sa new rdo para ipaupdate *** record ko. medyo hassle lang

Salamat sa information. Magkaiba yung RDO (Cubao ako naka register, Makati yung papasukan ko) so malamang gagawin ko yung mas complicated na process. Kung magpapaupdate may iba pa bang requirements na kailangan iprisenta (previous ITRs, IDs) o wala na, punta na lang ako dun?

Gano mo katagal ginawa? Mga dalawang araw ganun?
