HELP: Lost License

I recently lost my license, and I know that the ff requirements are needed to replace it:

- affidavit of loss
- photocopy of valid ID (TIN, NBI certificate) [I don't have a photocopy of my license or the receipt)

It was also supposed to expire this March (lost it in Feb.)

So my questions are:
- are there other requirements that I need apart from those stated above, given that the license is also set to expire on March?

- where do I get my replacement? Is any LTO office good enough (I was planning to get it at Cubao) or do I have to go to the main at East Ave?

If you guys have other tips as well as to how I can go about this, I would really appreciate it.

under duplicate license:

If License is due for renewal

Medical Certificate with Official Receipt from any LTO Accredited or Government Physician

Negative Drug Test Result from any DOH accredited drug testing center or Government hospital (for Driver’s License only)

If license is delinquent, applicant shall undergo the required examinations

i think as long as hindi licensing renewal center ang pupuntahan mo, no need to go to the main branch.

have recently rnewed my licnese, nagtry ask ako for a friend kung ano procedure, for lost license, sabi customer service ang process na daanan is for application ng new... kahit me affidavit ka... ewan ko kung ditu lang sa LP hndi nghuhukay ng records kaya new application sya... me seminar pa yun saka drive test....hehehe... kakadugo ng nose... pero kuha naman new ng same day...

lost my license din eto yung naexperience ko
- hindi pwede sa satellite office like yung nasa mrt ayala, pinapapunta nila ako sa main sa makati.
- sa paranaque lto na lang ako nagapply ng loss. vinerify lang nila kung me record na ako dun , i always renew at this branch. tapos affidavit lang tapos na. wlang medical etc...
