The answer to your question may be found in the New Civil Code of the Philippines.
Here are the provisions applicable to your situation:
Art. 1477. The ownership of the thing sold shall be transferred to the vendee upon the actual or constructive delivery thereof. (n)
Art. 1496. The ownership of the thing sold is acquired by the vendee from the moment it is delivered to him in any of the ways specified in Articles 1497 to 1501, or in any other manner signifying an agreement that the possession is transferred from the vendor to the vendee. (n)
1497 - actual delivery (physical delivery of the thing sold)
1498 - symbolic delivery (either made through a public instrument like the execution of a duly notarized deed of sale or in case of movable propery like a car by delivering the keys of the place where it is kept or stored)
1499, 1500 and 1501 are no longer applicable according to the facts you have given.
The general rule is that the ownership of the thing sold is transferred to the vendee upon delivery. The exception is when you and the buyer stipulated that ownership shall not pass until the purchase price had been fully paid. In layman's term, it is a sale of a thing on installment plan.
Hence, if you have delivered the car to the seller, whether by actual or constructive delivery, the ownership of the same has been transferred to him already.

Question:
1. i supposed that both the buyer and seller get a copy of deed of sale. who gets the original? or they need to have 2 original (for each of them)?
2. what documents should the buyer ask from the seller so the owner ship can be transferred? (aside from OR/CR, deed of sale)
3. should the buyer ask the seller to sign an acknowledgment that he already receive the money? or the deed of sale is enough.
4. after the payment, should the deed of sale be notarized first before the seller release the car to the buyer?

1. bring 3 copies of deed of sale during signing to make sure buyer/ seller has an original copy. the third copy will be used for the transfer
2. aside from OR/ CR, deed of sale, you need a photocopy of the seller/ buyer's ID
3. no need in most cases. deed of sale should suffice
4. not necessary to notarize. usually, car is released after signing of deed of sale and payment. notarization can follow after.

The car was purchased through company car plan and is registered as "<company name> assigned to <name of late spouse>" A deed of sale has already been executed between the company and the late spouse long before his death and this document, along with other pertinent documents, are now in the widow's possession.
