    to cathsy: not all afflicted with PTB who takes anti-PTB meds have scars in their chest x-ray. Some do heal, with clear lungs.

    to master_green: in the case that a surgery is needed, it is usually serious and risky. Having bullae in itself means that it is serious.

    ^ sabi sakin ng doctor your current medications would not be enough to treat you..then, you would need a higher dosage or a different medication which your body might not be able to tolerate anymore..

    ^di ba may possibility nga na maging drug resistant yung bacteria pag nag-skip ng gamot?? MDR ata tawag dun. pano nga kapag ganun kahit minimal ptb lang? saka pano naman kapag hindi naman nag-skip ng gamot kahit 1 day pero minsan nagkaiba yung oras ng pagtake for example kahapon eh 8am tapos ngayon ay medyo na-late?

    although hindi pa naman ako nag-skip ng med kahit kailan.

    what if i escape the medication for 2 days and there are times that i forgot to take one out of 3 med. im really bothered of what i red that if u dont take the medication the bacteria will be resistant to the antibiotic. im just worried..what if im taking my med now and then its useless since the bacteria is now resistant. i have a minimal ptb. help me pls.i dont have any signs and symptoms..but then when i started becoming an intern med.tech in a hospital..i felt like its hard to breath for me and i have a back pains.. thats the only considered signs i have..or maybe its just a psychological thingking because i work in a hosp. i just have my afb test this march,and it was negtive.should i take another afb test now?im worried..what if im now active.. help me please..

    Missing your schedule of your anti-PTB medicines can cause multi-drug resistant (MDR) PTB. No one really knows if you skip a day's dose or 2 day's dose of the medicine, you'll have MDR but by missing it, it can increase the chances of having MDR. That's why it is advised that you take your medicines religiously and regularly. If you skip a dose, then just take it as soon as you remember it and follow-up with your physician ASAP.

    hay sobrang busy sa work... i forgot to post na tuloy. well im so happy with my xray results. according to my doctor eh effective sakin yun gamot. my xray showed partial clearing of the upper left etc etc... comparing to the last xray eh may improvement naman nga ako nakita or parang im hoping for the best talaga. medyo luminaw kasi yun xray unlike dun sa company ape namin. parang mas nadetect ko talaga na may blurred part na konti. anyway my doctor is also happy so why bothered to be sad diba... hayz tuwing naiisip ko na lang kasi nasasad parin ako.

    anyway got a new prescription... after 2months of quadtab, i have to take tritab naman for 4months. anyway tataka lang ako kasi parang 2meds lang yun iba dito after 2months pero sakin 3... yah im still hoping parin na mawala yun haziness part sa upper left lungs ko. talagang im praying na when i graduate from this eh talagang clear na sya. medyo swerte na nga ko kasi may trabaho na ako and they dont want me to resign. yung manager ko eh very professional naman when it comes to my case parang confidential sya kasi wala naman ibang nakakaalam kundi ako at sya lang. basta i take meds daw and proper diet. he even put me in a temporary account para daw makapagpahinga ko. kaso yun temporary account na yun eh masnakakapagod pa sa hawak ko dati kaya ganun rin huhuhu. well im still thankful na upto now eh im still standing and moving on with my life. pero kinakabahan parin kasi ko... plan ko kasi magtrabaho sa singapore and medyo natatakot na ko baka kasi when i get there eh di nila ko tanggapin.

    @freeman... ilang months bootcamp yan? hehe nacurious bigla eh, i also did come from bootcamp 2 years ago. im still happy for you din na atleast by june eh makakapagumpisa ka na ng new life. sana everything would be okay para sating lahat.

    @master green... yup siguro nga nagkataon lang na ece tayo lahat hehehe. nakakailang months ka na? hays im thankful enough na may ganitong site na pwede ko ilabas lahat ng nasa isip ko. actually upto now sobrang konti lang ng nakakaalam na may ptb ako. im still scared na if malaman nila eh they will not accept me. emo hahah

    The bootcamp was supposedly 8 weeks but it was shortened to 6 weeks due to client demands. I don't know about the June bootcamp though.

    Doc Kram, I'm in a little panic now. I'm having some rashes in my arms. Is this a side effect of my medication (fixcom4) or is this my body's reaction to the ptb bacteria?

    kung meron na ba ng MDR, at hindi nagamot yung ptb for the 6months medication, ano yung gagawin para magamot pa? Or should I say 'Magagamot' pa ba? Do you need to be hospitalize na to undergo such intolerable series again of medication?

    ^ magagamot po ang mdr tb pero mas mahirap and mas complicated compared sa regular tb. MDR-tb patients will be taking second line drugs compared sa regular tb patient who's taking first line drugs. Mas madaming side effects and second line compared sa first line drugs.

    Ang tb patient kailangan tamang gamot ang iniinum kasi kahit araw araw ang inom or on time talaga, pero kulang naman ng dose, it's possible na maging resistant na.

    Pag mdr-tb hindi pa din nagamot at lumala, it will lead to xdr tb.

    ^ yes po and second line drugs may tablets,paser( parang maliliit na sago na madami tapos kailangan ihalo sa juice para mainom pero hindi natutunaw direcho lunok) and injectibles na like streptomycin or kanamycin. The numbers of the tablets will depend on your weight. example lang to (for a 76kg. person 27 tablets (mon-sat) hindi pa kasama paser(mon-sat) en injection(for 6 months) sunday pahinga naman ng katawan yun at walang pasok sa clinic. pinagmemeetingan ng mga doctors bago ibigay ang tamang gamot dapat hindi resistant yung bacteria sa mga ireresetang gamot. magbabase sila sa dst, sputum, culture). Tutuk gamutan ang mdr tb. Araw araw pupunta ka sa dots clinic para uminom nakaharap sayo ang dots partner para tiyakin na iniinum ang gamot.
    ang mdrtb 18 months to 24 months ang gamutan.
    Side effects: some are the worst side effects
    blindness, nabingi, sobrang sakit ng tyan, pagtatae, pagkahilo, etc.
    note: pag may nararamdaman kailangan sabihin agad sa dr. para maiwasan ang hindi magandang resulta. Pag nanlalabo ng paningin o parang may nadidinig na ugong sa tenga, etc.
    Ipagpatuloy po ng mga tb patients ang gamutan at kayang kaya yan basta tutok gamutan, The best ang DOTS.

    The daily dosage is usually calculated for this by body weight:

    71 kg 5 tab/day, 55-70 kg 4 tab/day, 40-54 kg 3 tab/day


    for myrin p forte po yang dosage sa taas which contains: Ethambutol HCl 275 mg, rifampicin 150 mg, INH 75 mg, pyrazinamide 400 mg (per tablet)

    now here's my concern:

    --->sa case ko po kasi, nung 1st month ng treatment, less than 55kg ang weight ko kaya 3 tab/day of myrin p forte ang tinake ko.

    --->nung 2nd month ng treatment, 56kg na ang weight ko kaya 4 tab/day na ng myrin p forte ang tinake ko.

    --->tapos nung start ng 3rd month (maintenance period na), tres pack na ang tinake ko 2 tab/day, 1 tab before breakfast which contains 450mg rifampicin+400mg isoniazid. the other tab is before lunch which is 1000mg of ethambutol. bale wala na po yung pyrazinamide sa treatment.

    --->i'm on my 4th month of medication at tuloy lang ang pagtake ko ng same dosage na sinimulan ko nung 3rd month for maintenance (which is tres pack). btw, 56kg pa rin ang weight ko ngayon.

    my question is, tama po ba ang gamot ko at ang dosage nito based sa mga info na ibinigay ko sa taas?? and ano po ang chance na maging resistant, if ever meron man, based on my daily dosage? or let say tama naman yung pagtake ko ng gamot, ibig sabihin walang possibility na magka-mdr ako?

    (btw, i never skip med so far even for a day. pero may particular instances na hindi on-time yung pag-take ko like twice or thrice na nangyari na na-late but still i never skip one.)

    sorry, medyo mahaba ang post ko at may pagka-OC ang dating and i know that these concerns should be addressed properly to my pulmo, but let me take the opportunity to share it with you and hear your thoughts about this thing, mdr-ish errrr. thanks po in advance.

    yung sa dosage for mdrtb patients ang nagcompute for that are the doctors sa dots, for simple explanation nalang po ang ibibigay ko pag matimbang mas madami ang gamot, pag magaan ang timbang konti ang gamot. Kailangan din ingatan ang liver kasi prone sa liver failure sa dami ng gamot na iniinom. So kung ano ang nararapat na dose sa isang patient based sa weight yun ang ibibigay. Pag ang matimbang na patient eh malaki talaga ang ibinagsak ng timbang pagmeemeetingan ng drs. kasi babawasan ang gamot. Kinukuha ang timbang every month para makita ang difference from the past month.

    ang gamot nyo po ay ethambutol, isoniazid, rifampicin, pyrazinamide tama po? Dr. nyo po ba ang nagsabi na alisin na ang pyrazinamide? Bakit po? Ask ko lang po bakit wala kayong streptomycin?( injectible)

    isoniazid:
    Ipaalam agad sa dr. kung kayo ang nakakaramdan ng alinman sa sumusunod: panlalabo ng paningin, pagdilim ng kulay ng ihi, pamamantal, paninilaw ng mata at balat, pamamanhid ng paa at kamay

    rifampicin:
    Ipaalam agad sa dr. kung kayo ang nakakaramdan ng alinman sa sumusunod: lagnat, panginginig, pananakit ng lalamunan, at paninilaw ng mata at balat

    pyrazinamide:
    Ipaalam agad sa dr. kung kayo ang nakakaramdan ng alinman sa sumusunod: pananakit at pamamaga ng kasukasuan, lagnat, hilo, pagsusuka, panghihina, pagdilim ng kulay ng ihi at paninilaw ng kulay ng mata at balat

    ethambutol:
    Ipaalam agad sa dr. kung kayo ang nakakaramdan ng alinman sa sumusunod: panlalabo ng paningin, pamamantal, pamamanhid ng kamay at paa, at problema sa paningin ng mga kulay

    Streptomycin: (itinuturok sa braso o puwit)
    Ipaalam agad sa dr. kung kayo ang nakakaramdam ng alinman sa sumusunod: pag ugong sa tenga, pagkabog sa dibdib, matinding pagkauhaw, pagsusuka, pananakit ng tyan, problema sa pngangati, pagbabago sa dami at dalas ng pag ihi at panghihina ng paa at kamay

    nagpapaculture po ba kayo ng sputum? Kasi doon binabase kung negative na kayo (hindi na nakakahawa). Chaka sa sputum test doon malalaman talaga kung gumagaling kayo. For mdr tb patients naman, it's a must na every month mag submit ng sputum for testing sa dots clinic para malaman kung negative na. Every after 6months ang xray.


    Man-made po ang mdr tb.
    1.Resulta ito ng maling gamutan.
    2. Pag ang isang tao nahawa sa isang mdr tb patient, ang maisasalin niya automatic mdr tb din hindi regular tb.

    Mabuti po hindi kayo nag iiskip ng kahit isang araw sa gamutan. Yung pagkalate po ng intake nyo ng gamot sana hindi nagkakalayo sa tamang oras ng pag inum nyo po.

    These are my insights lang po, kindly consult din sa inyong dr. po. Sana po nakatulong.

    Generally po ang mdrtb patients hindi na nagwowork sa sobrang dami at tindi ng side effects hindi na nakakayanan magtrabaho. May iilan na mdrtb patients na nakakapagwork like sa office kasi aircon naman chaka hindi nagbubuhat ng mabibigat. Yung iba nakakaya ang tindi ng tama ng gamot, may iba na hindi po talaga kaya.

    ganun po ba? Nakakatakot naman pala ang mdr tb.

    Nasa 6months phase na kasi ako. But i feel hardness in breathing sometimes pa rin.. Pero wala ng fever, night sweats, panghihina, at iba pa na naramdaman ko dati. Yung lang hirap sa paghinga at back pains paminsan-minsan.

    Worried na ako na candidate na ako to MDR TB, huwag naman sana. All my sputum results were negative bago at habang umiinom ako ng gamot.

    thanks po sa info. btw, yung impression/diagnosis po kasi sakin eh minimal ptb lang (class 3) or latent tb ba ang tawag dun, and asymptomatic din ako, and medyo clear din yung film ng xray ko, like parang walang spot. (actually marami po kami na minimal ptb ang case sa thread na ito.)

    about dun sa gamot, di ba po 6 months yung medication, 2 months yung intensive and 4 months yung maintenance. dun sa 1st 2 months na intensive, myrin p forte (ethambutol, rifampizin, inh, pyrazinamide) yung nireseta sakin. tapos nung maintenance period, tres pack naman ang nireseta, eh wala namang pyrazinamide sa dosage ng tres pack.

    yung streptomycin(injectible), di ba po sa mga active cases na yun o yung sa medyo malala na?? kaya po siguro wala kaming streptomycin kasi minimal lang yung samen.

    so far wala naman akong naramdamang side effects or kakaiba sa katawan ko, except dun sa orange na ihi and yes madilim minsan but not too dark (yun na siguro ang epekto sa liver ng gamot) and yun nga, nasa 4th month na ako ng maintenance, at gusto ko na talaga tong matapos until the 6th month, pahirap na rin kasi, (frustrated, hehe)

    btw, effective na diet po ba ang apple at milk for ptb?, sa simula kasi ng gamutan ko yun ang madalas kong kinakain, tapos twice a day yung milk, surprisingly nag-gain ako ng weight.

    Para malaman din na gumagaling na, nawawala yung mga dating karamdaman like pabalik balik na fever, hinihingal, night sweats chaka yung tunog pag humihinga pag natutulog,etc. pero hindi ibig sabihin na umaayos na magaling na talaga, meron po kasing iba na nag gagamot pag nakaramdam ng ginhawa ititigil na akala magaling na..kaya tuloy posibleng maging mdr nagiging resistant na bacteria. kailangan pa din na matapos talaga ang gamutan at icheck ang sputum test.

    Bakit nyo po naiisip na baka candidate kayo for mdr? may nararamdaman po ba kayo na kakaiba?

    Kung negative po kayo sa sputum test(culture, smear) tapos ok din ang xray, tapos you're taking the meds religiously, I believe there's nothing to worry po Congrats po!

    ask ko lang po hindi ba required ang sputum test sa latent tb patient? saan po nagbabase kung cured na?

    yung streptomycin po kasama sa 5 first-line drugs. ang patient na may regular tb lang kailangan matake ang ethambutol, isoniazid, pyrazinamide, rifampicin, streptomycin.

    Baka nga po na dahil latent tb lang po kayo kaya walang strepto? Pakiask nalang din po sa dr. nyo para maclear.

    Share ko lang po sa dots clinic regular tb lang nag iinject din. Hangga't nasa regular tb pa lang kailangan ng patayin ang bacteria kumbaga hindi binebaby hindi na pinapaporma kasi first line drugs ang pinakamabisa sa lahat ng gamot sa tb. Second line drugs less effective, matagal gamutan at madami side effects.

    Mabuti po at wala kayong masamang nararamdaman, sa ihi lang talaga orangey ang kulay. Pag mdr tb po kasi ang mga gamot
    araw2x ramdam ang tama ng gamot.

    yung about po sa milk, meron kasing gamot na hindi compatible ang milk, nababawasan ng bisa ang gamot example quinolones( moxifloxacin)kung iinum ng gatas o multivitamins kailangan magpalipas ng 2 oras bago uminom ng moxi. Moxifloxacin ay second line drug na po na iniinum ng mdrtb patient.
    Since wala po kayong moxi maaaring ok naman po ang gatas.
    Kailangan din limitahan ang pagbibilad sa araw pag umiinom ng moxi kasi ang epekto para kang magcocolapse dahil sa reaction.
    Mayroon ding gamot na pag tinake mo like cycloserine (2nd line drug) kailangan sabayan ng vitamin b6 para pangontra o maiwasan ang side effect ng cycloserine.

    Eat lang po ng masustansyang pagkain, gulay and prutas. Ang pyrazinamide nakakataas ng uric acid kaya hinay sa gulay na pampataas ng uric acid like beans etc.

    Mabuti po malapit na kayong matapos ng gamutan, tuloy tuloy lang po! kayang kaya yan
