    Hi Doc, tanong ko lang po kung para saan ang powerchain na rubber. Ganun na po kasi ang rubber na ginamit sa akin ngayon lang. Nabasa ko po na ibig sabihin patapos na ang paggamit ng braces kapag ginamitan na ng powerchain. Tama po ba ang nabasa ko? Thanks!

    hi doc smile! question lang po... kc *** Ortho ko more than 2 years years na nya ** patient, we started from irritating mouth-guards, wisdom tooth extractions, finally ceramic braces,which did not last long, hihihi bad habits never die~she changed it to the metal ones (ordinary brackets), which i'm perfectly fine and i trust that she knows what she's doing. but recently my cousin had her braces done by another ortho just recently, i notced she's wearing colourful rubber bands, those small ones placed on each bracket. my ortho never placed anything like that on my brackets, instead she uses a very thin wire to loop around and lock the arch wire (i dunno what its called exactly) and clear powerchain which turns yellowish if i eat curry based dishes. i'm going to ask her about these on my next visit, i just want to know your opinion on this.

    so sorry for the very long narration.

    Space closure is usually the middle part of the braces treatment. Malamang nasa kalagintnaan pa lang kayo o bandang simula pa lang.

    Before space closure, teeth need to be straighthened first. After space closure, the positions of your canines and your molars are optimized.

    It's true, ceramic braces need to be well taken cared of, if not you're really better of with metal braces.

    The orthodontic archwire can be affixed to the braces with either colorful elastic modules or metal ligature wire. The advantage of using metal ligature is that it attracts less dirt ( mas dumihin kasi yung elastic module ) and treatment progresses faster because of the intimate adaptation of the wire to the brackets. Colorful elastic modules can be used effectively for orthodontic treatment. They do need to be changed every month though, unlike ligature wire which can last several months.

    The reason your treatment provider used clear powerchain is because you used to have ceramic braces. Your braces will look strange if you had clear brackets and colorful elastics. You will look silly. With metal brackets it's okay to use colorful elastics.

    ^ I charge P100,000 minimum or higher for Damon. Some orthodontists charge higher. Damon 3mx (metal) and Damon Q (metal) are already available here in the Philippines. Damon Q Clear will be available next year.

    Damon is best used for nonextraction cases (with the exception of the removal of third molars, although it can be used in several extraction cases depending on the situation ) in which the patient's profile can afford a little bit of protrusion.

    Mas nagiging matambok kasi ang nguso ng pasyente with Damon, because it's effect is to expand the teeth and move them forward. Kung matambok na matambok na ang nguso ng pasyente to begin with, using Damon may not be the best option. Kung lubog naman ang nguso ng pasyente magandang gamitin ang Damon.

    Damon brackets offer passive self ligation, with lesser friction which allows the use of lesser forces ( therefore lesser discomfort ) and potentially faster movement of teeth.

    ^ Para makabit kasi yung mga bracket, kailangang tanggalin yung sobrang pandikit na nakakapit sa mesh nila para malagyan ng bagong pandikit at makabit ulit. We use either heat or a sand blaster to remove the excess adhesive. The darkening of the bracket will fade in time, as long as you continue brushing them gently after every meal.

    For my patients, when brackets have fallen off twice I actually don't use them anymore and replace them with new brackets. I charge the patient from P1,500 to P3,000 per bracket replaced, depending on the type of bracket replaced.

    Aside from making sure that the orthodontic archwire is well adapted to the bracket ( necessary for optimal tooth movement ), metal ligature wire can be used to keep teeth together ( which is important once spaces have been closed completely, as these spaces tend to reopen ). They can also be used to control and make minor changes in Arch length.

    Elastomeric chains , while mostly used for closing spaces, can also be used to derotate teeth, to somewhat affect the torque of anterior teeth and to a lesser extent even the posterior teeth, and make changes in arch length and the Curve of Spee.

    I'll be having my braces this week. My ortho wants to schedule appointments every 2 weeks since this will be more cost efficient for his clinic daw. Isn't that too soon? My friend says that it's too soon and my teeth wouldn't have adjusted by then? Or is it just fine?

    ^ For strap-up (meaning placement of braces and attachments) appointments it's okay to visit your treatment provider every week. This means for the 1st month of your braces you'll be visiting your treatment provider more frequently. From the 2nd month onwards, assuming all the attachments are placed and your teeth start are already moving, ideally the adjustment for braces should be every 3 weeks or every 4 weeks or depending longer. In between adjustments your teeth and the tissues surrounding your teeth need to "recover" before the next adjustment.
