please help...

:confused: what do i do? what are the things to submit to the LTO? :confused:

thank you in advance for your kind help. i really need a new one...

If you want to be absolutely sure of getting a new one, go to a lawyer or notary public and execute an "Affidavit of Loss," which is an official, notarized document stating that you lost your license, and explaining the circumstances behind the loss. The government won't just issue new licenses on a whim, so you have to at least convince them that the license was truly lost (as in a hold-up or a wallet theft) and not merely misplaced.

After you get this, proceed to the LTO to get a re-issuance of your license. It would help if you had a copy of your old license (photocopy would be best), or at the very least, the pertinent details, such as License number, exact date of issuance, etc., etc.

Also, please try to get it from the LTO office you initially applied to. This would help expedite things, as getting it from a far-away LTO office may cause delays.

my driver's license, i assume, fell off when i was at the church. then it rained. it rained so hard. i just realized it was lost after about 6 hrs after going to church.

nope. i dont have a photocopy of my license and i don't know my license number. all i know is the date off issuance.

how do i do it? i just go to a notary public, i'll just tell him how i lost it etc etc. then i get to swear in, then he makes the affidavit of loss then i have to give it to LTO then they'd give me a new one.
was that right?

Yup, that's how you'd do it. There are plenty of notary publics around the LTO in East Avenue, for example. Just approach one and ask how much it costs to execute an affidavit of loss. make at least two other copies, so you have a copy yourself.

It's ok if you don't have your number, but be prepared, especially if your LTO is a small branch, to possibly search for it for a while. Good luck! :)

Yeah to add to what Aticus said, it would really help if you had the paper temporary license. When I lost mine, fortunately, I still had the paper license.

From there, they make YOU trace your own license. (at least that's what they told me..) The paper licenses are arranged in these size-2-sized (yes the ones we used to use in Grade School...) cartolina bound stacks of duplicate paper licenses, arranged by DATE.

Luckily for me, I also knew the date when I renewed my license. December 19, 1998. So it was just a matter of looking for the bundle that said "December 1998". From there I showed them their copy (which I know realize makes me ask the question, why do they need ME to find the DUPLICATE copy which is EXACTLY like the paper license THEY themselves issued me?) :lol:

Oh well. After that, once you prove to them that you lost it (affidavit), and they check your paper license with the paper license they have on file, they get your OLD paper receipt, stamp it, keep it (so now they have 2 copies) ask you to fill out a new one, and then you go back for it in 2 weeks.

Here's a tip then:

XEROX all pertinent ID's you have in your wallet. Make a photocopy of your:

1. Paper Driver's License
2. Actual Driver's License
3. SSS ID
4. Credit cards? - Maybe just know where to call to cancel - that should be enough.

SO just in case you lose your license again, it's a much simpler process to track it down.

If I HADN'T know the exact date when my license was first issued, I would have had to sort through EACH and EVERY one of the paper licenses (oh about 200 per bundle), and all the bundles put together, formed PILLARS and PILLARS of bundles from floor to ceiling, left to right, of an entire wall, of an entire room of the PILOT LTO.

actually noisy cricket... what i lost was the paper license. coz i haven't gotten the permanent one yet.

my LTO branch is in Las Pinas. it's kinda small though.

by the way, thanks. ill get my affidavit of loss maybe on monday or saturday.

I lost mine a year ago, kasama ang prc license ko.I secured an affidavit of loss documenting the incident and I immediately reported it to LTO main office, parang isusurveillance kasi ang lost license mo because someone stupid might have used it already.After giving me a clearance, i was instructed to go to the branch where my license was issued and presented the clearance from main office.I went through same process-parang nagrenew ka lang, plus the affidavit and clearance nga lang. Just be patient.Makakapagdrive ka pa rin uli pards.Mas mahirap ang mahuli na walang license.
